Why does medical insurance cover psychiatry care but not psychotherapy?

Unless you go to a psychiatrist who puts you on meds, the regular psychologists are not covered by medical care. Is that the case with all insurance providers? I can’t find one that covers psycotherapy by a PHd and not an MD therapist.

I went to my primary physician a couple of years ago, and he recommended me to the "group" sessions Kaiser Permanente offers. It has proved to be useless, and did not help me at all. I’m looking at other treatment options (individual psychotherapy/cognitive behavior therapy) outside my insurance…Will it cover at least some of it?
